Title: Non-elliptical scar revision techniques - A case series

Introduction:
Facial scars, particularly post-traumatic ones, pose a considerable aesthetic and psychosocial challenge. Innovative incision techniques allow improved cosmetic outcomes by camouflaging scars along relaxed skin tension lines and minimizing loss of normal tissue. We present three cases of post-traumatic facial scars managed using various modification techniques of scar revision.
Objective:
The objective of this study was to evaluate the efficacy of surgical modification techniques in improving the aesthetic appearance of post-traumatic facial scars.
Methodology:
Three patients with post-traumatic scars underwent revision surgery. Patient 1 had a linear scar (1.9 × 1.2 cm) over right malar region, revised with geometric broken line incision and closure using half-buried 5-0 PDS sutures. Patient 2 had an oval scar (1.4 × 1.3 cm) on right cheek, revised with checkerboard incision excising diametrically opposite sectors, closed with S-shaped closure along RSTL. Patient 3 had an oval scar (2.1 × 1.1 cm) on left cheek, revised using checkerboard incision.
Results:
At more than 5 months of follow up, all patients showed significant improvement in cosmetic outcome. In Patient 1, scar reduced to 1.9 × 0.5 cm, with approximately 58% decrease in scar area. In Patient 2, scar decreased to 1.3 × 0.8 cm, with approximately 43% reduction. In Patient 3, scar reduced to 1.75 × 1.0 cm with 24% reduction, with better alignment along RSTL. No major complications such as wound dehiscence, hypertrophy were observed.
Conclusion:
In scar revision, modified techniques improve cosmetic outcomes by placing incisions within natural skin creases and minimizing removal of healthy tissue. Non-elliptical incisions also prevent the need for creating an excision length three times the scar width.
